Subregional Functional Connectivity of the Precuneus as a Preclinical Biomarker in Alzheimer’s Disease. Neurology Research International, 7281786, 2026 [pdf] Abstract Introduction: The precuneus(Pc)isacentralhubintegratingdiversebrainfunctions, anditisearlyinvolvedinAlzheimer’s disease (AD)pathophysiology, yetitssubregionalvulnerabilityremainspoorlyunderstood.Thisstudyaimstodelineatethe diBerential involvementandvulnerabilityofthesesubdivisionsintheearlystagesofAD. Methods: We conductedaresting-stateFCanalysisofPcsubregionsin32cognitivelyunimpairedcarriersofthePSEN1E280A mutation comparedto25noncarriersusingseed-basedanalyses. Results: As comparedtononcarriers,wefoundthatthegroupofasymptoticcarriershadincreasedconnectivityfromthe7Am subregion towardtheentorhinalcortex,superiortemporalgyrus,insula-operculum,dlPFC,andsomatosensoryareas.Moreover, the POS2subregionsigniFcantlydecreasedconnectivitywiththeanteriorinsulaanddlPFC.HigherMoCAscorescorrelatedwith increased connectivitywithinandbetweenthePcandfrontoparietalnetworks. Discussion: These resultsrevealdistinctpatternsofearlyfunctionalreorganizationandsupportthesubregionalPcanalysisin identifying sensitivebiomarkersforpreclinicalAD,oftenoverlookedinconventionalwhole-regionanalyses.
